If your information is incorrect in the MDTC Member Directory, please complete the form below.

It is only necessary to fill out the NAME fields and then any contact information that must be corrected. If you would like something removed, such as an email address, put "remove" in the field and that data will be deleted from the online directory.

 If you would like your email address removed from the directory, enter REMOVE in this field.
